You will need to begin your name change notification process with the Social
Security office. Other government agencies and companies, including your employer’s payroll department, require that their files contain the same information as the Social Security Administration. The Social Security office will require an original, certified copy of your marriage license as well as a completed SS-5 form. Certified copies of your marriage certificate are available at the clerk’s office of the county in which you were married. The SS-5 form, or Social Security Administration Application for a Social Security Card is available at Wedding Decisions, 1404 West Kenosha, Broken Arrow. Call (918) 249-2345 for hours of operation. Mail the completed form, and the original certified copy of your marriage license, and, if applicable, proof of U.S. citizenship or lawful alien status, to the nearest social security office. You can find the address by calling the Social Security Administration office at 1 800 772-1213 or visit www.ssa.gov. The Social Security office will return your marriage license.
